What do I need to read DTC from a Transit 2,4 diesel auto 2003, DTC from both engine and transmission if possible.

If you're lucky it's EOBD compliant, although that is not required by law until 2006.
Possibly autoengenuity has support: OE-Level Coverage (Enhanced Interfaces) (http://www.autoenginuity.com/products-software.html#EI01)
Otherwise it will be Ford equipment.

ideally dealer tool - IDS

this will give best results

otherwise most generic tools will read codes but to a varying standard and acuracy

tools like x341, autodiagnos, delphi ds100e / ds150, carmen scan / wii, gendan, etc etc

Would IDS be able to update the ASM software ?
Only original or clone too ? Have heard about clones messing up the ASM ecu.
