Format in dos start>run>cmd>enter

format /q /fs:fat32 (Your drive Letter): for fat 32
format /q /fs:ntfs (Your drive letter): for ntfs

You can find your drive letter in disk management.
Make sure you choose the correct letter for your flash drive and not you HDD, check and double check.

Is the drive write protected?
Is it the correct drive letter?

You could try a live disc (DBAN or GParted) but usually not necessary. Failing that you may have a faulty drive. Try to format on another pc also to rule that out.

delete partition in disk management, create new partition. drive is in raw state.
Have read "active kill disk" should sort it but i've never tried it, worth a shot. I have used DBAN and it works well.
