Anyone got a copy of what a doctors sick note looks like?

Had a couple of days off work last week and they are asking for a note from the doctors now.

Ello mate, a doctors sick note is a sort of pre printed form that he/she would fill out as to say if you are fit for work or not.
I dont think that you could blag it sorry mate.

Also read that they shouldn't ask for one if I was off work for 7 days or less.

Correct, your employer can't ask for a doctor's sick note until you've been off seven days now. (it used to be three)

you self cert yourself for the first 7 days. its not a sick note anymore its a fitness for work note.

If you boss don't beleive you was off-line then tell him to shove his job, also for the time you was off, did you post on here?

Your boss is (was) entitled to bang on your door and check you really was ill.
