There are alot write ups on how to get a second iTunes account online but I thought I'd do one... Using iTunes10
1 - From desktop open iTunes
2 - Goto iTunes Store
3 - Scroll to the very bottom of page
4 - 4th column along under "Manage" press "change country"
5 - Select your country of choice
6 - Locate any free app to download
7 - Press "Create New Account"
8 - Fill in all required fields......
.co.uk email address are ok to use*(1 email account=1 iTunes account)*
9 - When you get to the page requiring payment methods/credit cards select "None"
10- Verify your new iTunes account in your email inbox.
11- "Authorise your computer" in iTunes under the Store tab. You should then be able to sync any idevice with any purchases made in iTunes store
The reason why I have more than one iTunes account is mainly because there are alot of apps that are specific to a region and cannot be got, in your particular country, e.g Pandora Radio-ok to download in USA but not in UK(only as a podcast)
