Any fault codes? All OK with rail pressure?
Could be problem with EGR or air mass sensor.

I'd go with Larue's suggestions to start with, from the info you've given they sound likely culprits.

Without error codes youre suffed. Many elecyrical isses also cause similar. Look for chafed wiring behind thermoplungers, behind engine under airbox and check good contacts on large blue plug and smaller black one in main fuse/relay box.

    Hi,
In 04 it could have an electric pump in the tank, has it got a hand primer beside the filter? If there is no primer there's an electric pump. Make sure its working. They will run without it but hard started.

Had one the with same symptoms couple days ago.Turned out leaking safety valve in fuel rail. It's located on the end of fuel rail and there is a leakback pipe on top. There should be no fuel at all coming out from rail under cranking or running. If there is fuel coming out from that pipe then replace valve.
As you haven't mentioned which engine management system it is i assume that is EDC15C...
