I know upa has problems with some ST95020 chips maybe also with ST95040. Clean the legs and read with another programmer.

you can read whitout probleme but for write sometine UPA canot do eeprom ST!!
remove the eprom an read in external adpater or soic pomona connection
use another eeprom atmel for write if you have a problem
