Default UPA - USB v1.3 read error (ST95640)

Tried anything but still the eprom cannot be read. it said vdd/vcc out of range
    tried

    a) using the test clip
    b) individually 8 pin miniature connector
    c) 4,5,6,8 connection
    d) J1 jumper
    e) external 12V supply

do I need to take out or desolder the ST95640 and read it 'ouside' to make it work?
