[HELP PLEASE] Exception Program Occurred

When i'm trying to open Usb Loader Gx there'll be a black screen with a lot of codes. first it will be "Exception (DSI) Occurred" and then restarting in 10s it changed to "Exception Program Occurred" and it keeps restarting in "Exception Program Occurred", i've searched in many forum(include this forum) but i still don't understand what should i do. Please anyone help me to fix this problem. and how to check my system information?

Post a sysCheck19 report as instructed I that link so I can see which IOS's you've installed.
