i know it sounds strange but breathe on disk till it mists up then wipe it ... it has worked for me lots of times ..

Your lens could be knackered, If you got it softmodded, why not get a hdd and usb loader, then you wont need the lens, unless of course you wanna rip through usb loader. Or replace the lens.
