For a standard Blackberry you would need to do the following...

Menu > Options > Advanced > SIM Card.

Type - MEPD (nothing will show on screen if there's no SIM inserted)

    Type - MEP2 (keystrokes are M E P 'Alt' 2)

This would bring up the MEP2 code entry box. Type in the MEP2 code & press select. Make sure you are making no errors in the entry of the code.

If you insert an accepted SIM before you start, a screen will be shown after you type MEPD. This will show which code is needed.
If it's MEP4, then you will need to type MEP4 instead of MEP2 before entering the MEP4 unlock code.
Sounds obvious, but the amount of people who overlook this is amazing.
