Not sure why the MEP2 code is so short. But I think you'll be needing the MEP4 code given your location.
Try the following with an accepted SIM card inserted....
Menu > Advanced > SIM > type 'mepd' (without the quotes)
If it works it will show you which SIM lock is being used. If MEP4, type mep (alt) 4 & enter the MEP 4 code in the box.
Phone asked me the unlock code right after the boot and didn't tell whether it wanted MEP2 or MEP4, it just said "Nework unlock code". So entered MEP4 first, nothing happened (well, counter changed form 255 to 254), then I entered MEP2 and it worked!
So thank you very much for your help! Now I could use this phone as a perfect test equipment for our GSM development.

2nd.. Do you have '0 attempts left' showing in the MEP(ALT)2 code box?
Both myself & Teerak2uk have provided the same codes, so something has to be wrong with either the details you supplied, the way you're using the codes or you have ran out of attempts.
Follow this....
Menu > Settings > Advanced > SIM
Type - M E P D
This should show you the SIM lock status for all 4 locks. If any of these show 'Active' you need to enter the corresponding code.
So if MEP2 is active you type - M E P (ALT) 2
Then enter the MEP2 code 9060845040065410
If it shows MEP4 active you type - M E P (ALT) 4
Then enter the MEP 4 code 2330858104442282
It is unlikely to be the MEP4 code as this is only used in the USA.
If you have 0 attempts left there's nothing anyone can do, except blackberry.
