The diode goes in series with the centre conductor. The end with the band around it goes to the LNB end.
The outer screen also needs to be reconnected, ensuring that it doesn't short out to the diode or capacitor leads.
I suppose the neatest way to do it is to mount a couple of F type sockets on a small metal box, with the components mounted inside.
EDIT...the capacitor shown in my original diagram is not required. I initially thought that the 22kHz tone was sent as a seperate signal, but it's the voltage supply to the LNB thats varied by 22kHz, so no capacitor needed.
